The sauerkraut you find on most supermarket shelves has been pasteurized. That means it's been heat-treated to extend shelf life and ensure consistency. The process works very well for those purposes.
But heat changes things. The microbial activity that develops during fermentation doesn't survive it. What remains is a product that tastes acidic and sour, but whose living characteristics have been altered significantly. It's preserved food, not living food.

What actually happens during raw fermentation

Real sauerkraut starts with two ingredients: cabbage and salt. That's it.
Salt draws water out of the cabbage through osmosis, creating a brine. In that environment, naturally occurring bacteria on the cabbage leaves begin to ferment the sugars present in the vegetable. No starter cultures are added. No vinegar. The process is entirely driven by what's already there.
Over roughly two to three weeks, the microbial community shifts. Lactic acid builds up, the pH drops, and the environment becomes inhospitable to unwanted bacteria. What you're left with is a food that has been chemically transformed by microbes, not just preserved.
Research published in the European Journal of Nutrition found that naturally fermented vegetables retain a distinct microbial profile compared to their pasteurized counterparts, with lactic acid bacteria present at levels that simply don't survive heat treatment. A note on what "real" means

It's worth naming the pattern directly. The word "fermented" on a label doesn't tell you whether the product is raw or pasteurized. Vinegar-based pickles are often marketed alongside naturally fermented ones. Sauerkraut made with added vinegar for speed looks identical to the slow-fermented version.
Reading the label carefully matters. Raw, unpasteurized, and naturally fermented are the terms worth looking for. If the product needs refrigeration from the moment it's made and has a shorter shelf life, that's usually a reasonable indicator that the fermentation is still active.
The version most people grew up with was convenient, long-lasting, and shelf-stable. Real sauerkraut is none of those things in the same way. It requires a fridge, has a shorter window, and behaves like food that's still alive, because it is.
That's the actual difference. And it's a bigger one than most people expect before they taste it.
